Dans le Londres de 1969, Withnail et Marwood sont deux comédiens sans succès qui partagent un appartement insalubre. En attendant de décrocher un grand rôle, les deux amis passent leurs journées à boire et à fumer. Quand enfin ils se décident à partir à la campagne pour sortir de ce cercle vicieux, ils atterrissent chez Monty, l’oncle de Witnhail. Homosexuel bedonnant, celui-ci jette aussitôt son dévolu sur Marwood.
